Service Directory Template

Introduction

One of the first tasks in the mapping exercise is to identify the services your organisation plans to map and create a service questionnaire for each of these services. The first page of each questionnaire asks you to provide contact information for the service. We are aware that local authorities already have local children’s service directories and to avoid duplication of effort we have developed a template that can be populated with information from your directory. This can then be uploaded to the mapping website to create your service questionnaires and populate some of the fields on the ‘contact’ page.

Following the completion of the exercise our service directory tool reports this data on our publicly available website. This can also be linked to your organisations’ website.

How to provide service directory information

An excel spreadsheet template is available below. This contains the fields that we can upload for you. Each row in the spreadsheet will represent one service/team and you will need to input information about each of the services/teams you plan to include in the mapping exercise.

Once complete please save the file (including the name of your organisation) and send to help@childrensmapping.org.uk. This will then be uploaded to the website and when you log on you will see each of your services on the ‘List Service Entries’ page.

Please note that each service/team you plan to map should only be included once so you will need to review the information on the spreadsheet before sending it to avoid duplication. Only include those services that can be mapped within the following categories:

  • Child health in the community services

  • Children’s hospital services

  • Maternity and neonatal care services

  • CAMHS (specialist tiers 2-4)

  • Family support services

  • Children and young people’s welfare services

  • Education and learning services

  • Services for young people

  • Children’s leisure services

  • Children’s central support services
