Help for Edit Service:
Practice questionnaires are designed to let you play with a questionnaire without having to register or enter any real data. The details you enter are only stored temporarily and are not held in the service mapping database. There are practice questionnaires for each of the different questionnaires:
- 2009/10 sample service questionnaire - Team/service questionnaire for all services
- PCT commissioner questionnaire - An organisation-wide questionnaire about PCT commissioning arrangements for completion by PCT commissioners
- PCT commissioner safeguarding questionnaire - An organisation-wide questionnaire about PCT safeguarding arrangements for completion by PCT commissioners
- NHS provider questionnaire - An organisation-wide questionnaire about safeguarding and breastfeeding arrangements for completion by NHS providers, including PCT providers
- Children's trust questionnaire - A questionnaire about Children's Trust arrangements, to be completed by local authorities on behalf of the Children's Trust partners
- NI51 - A questionnaire for local authorities containing the National Indicator 51 questions - Effectiveness of Child and Adolescent Mental Health Services (CAMHS)

On this page you are asked to provide contact information for the particular service/team you are mapping. These include the name of the service/team, postcode and a number of local service directory fields such as the address, phone number and how the service is accessed.
Tips for completion
The name of the service/team is an important identifier on the mapping database. You will use the name to identify the services/teams you are editing. The service/team name is also listed on a range of reports. In order to make the reports more useful, locally, regionally and nationally the name should include both an area and service type identifier. For example, Durham West Health Visiting is a more useful name than just Health Visiting given the potential number of Health Visiting services across the country.
New for 2009/10
There are some new local directory fields including website address; facilities and language spoken by staff in the service.
The option to add delivery locations. Entering these details will mean that the other locations will appear on the service directory map.
